Can Ankle Sprain Can Lead to a Calf Tear?
As a rehab and movement specialist, we view ankle sprain as a movement system injury, not just ligament damage. While the injury starts at the ankle joint, its effects often travel up the kinetic chain. When ankle stability, mobility, and proprioception (self awareness of the joints in space) are impaired, the body compensates — shifting load to the calf muscles, especially the proximal gastrocnemius, also known as the upper calf – located behind the knee.
A common misconception is that once ankle pain and swelling resolve, the injury is healed. In reality, unresolved instability and altered movement mechanics often remain. This leads to poor push-off mechanics, early heel rise, reduced shock absorption, and increased calf overload, which significantly increases the risk of an upper calf tear during running, jumping, dancing, sprinting, or directional changes in lateral and rotational sports such as: tennis, basketball, baseball, soccer.
